Health insurance plans will often cover nutrition visits at no cost to you.
Telehealth is covered by most plans especially due to the expansiveness of our state.
FFTN Nutrition is a preferred provider(in-network) for:

Premera Blue Cross
First Choice Health
ββAll insurance plans are different in what they will cover. It is strongly encouraged you determine your coverage prior to appointments. Coverage is determined by calling your insurance plan. Coverage can never be guaranteed.
Insurance benefits are not verified by our office prior to your appointment. FFTN is in network with many plans but not all plans pay for Medical Nutrition Therapy.
For insurance, it is important to ask:
-What are my nutrition benefits?
-Is nutrition counseling covered? (share these procedure codes 97802 & 97803, S9470)
-Are there restrictions on which diagnoses are covered?
-Is Z71.3 (dietary counseling and surveillance) covered, or what medical diagnosis are required to utilize my nutrition benefits?
-Do I need a referral or prior authorization?
-Is nutrition counseling covered when provided via telehealth?
-How many visits are allowed for my benefit year?
-Are out of network nutrition providers covered?
-Will nutrition services count towards my deductible or out of pocket fees?

FFTN is happy to accept insurance but it is important that patients are aware of their policy benefits to try avoid surprises when it comes to fees and coverage. Please let me know prior to your appointment if you have questions or concerns regarding coverage.
Physicians Referral
Premera, BCBS, Cigna, First Choice Health-do not typically require a physicians referral
Aetna -physicians referral is required prior to appointment.
Ways to pay:
FFTN accepts cash, check, credit/debit cards, HSA & FSA cards.
A credit/debit card is required to be on file to secure appointments.
